Marionette Lines Treatment

Marionette lines are one of the concerns I hear about most in clinic, usually described in a single sentence: "I look sad or cross when I'm not." Clients point to the lines running from the corners of their mouth down towards the chin and ask whether anything can actually soften them.

The honest answer is that it depends on what is causing them. Marionette lines are rarely just a wrinkle. In this guide I'll explain what causes them, which treatment approaches exist, what the risks are, and why the right plan starts with an assessment, not a syringe.

What are marionette lines?

Marionette lines are the vertical folds that run from the corners of the mouth down towards the jawline. They take their name from marionette puppets, whose jaws move along hinged lines in the same place.

They are different from nasolabial folds, which run from the side of the nose to the corners of the mouth. The two often appear together, but they have different causes and need different thinking. If the nasolabial area is your main concern, my post on overfilled nasolabial folds explains why more product is not always the answer there either.

What causes marionette lines?

In my experience, marionette lines usually come from a combination of these changes:

  • Volume loss. Fat pads in the cheeks and around the mouth shrink and shift with age, so the tissue above the lines loses support.

  • Bone changes. The jaw and chin gradually lose bone density, which changes how the soft tissue sits.

  • Collagen and elastin decline. Thinner, less elastic skin folds more easily and holds creases for longer.

  • Muscle pull. A small muscle at the corner of the mouth pulls downward. In some people this pull is strong enough to deepen the line and turn the mouth corners down.

  • Lifestyle factors. Sun exposure, smoking, rapid weight loss and genetics all play a part.

This is why two people with lines that look similar can need completely different plans.

How I assess marionette lines

Before recommending anything, I look at the whole lower face, not just the line itself. I check how the cheeks are supporting the area above, whether the mouth corners turn down at rest, how the line changes when you talk and smile, the quality and thickness of your skin, and the shape of your jawline and chin.

We also go through your medical history, medications and any previous treatments. Sometimes the most effective change is made away from the line itself, for example by restoring support higher in the face. Sometimes the honest recommendation is skin treatments only, or no treatment at all.

Treatment options for marionette lines

Every option below has benefits and limitations. Results vary from person to person, and no treatment can promise a specific outcome.

Volume replacement with dermal filler

Hyaluronic acid dermal filler can be placed to restore lost support around the mouth, chin or cheeks. Used conservatively, it can soften the depth of the fold and reduce the shadow that makes the face look downturned. Hyaluronic acid filler can also be dissolved if needed.

It is not suitable for everyone. Overfilling this area can look heavy and unnatural, which is why I prefer to build gradually. You can read more about my approach on the Fairfield volume replacement page.

Collagen-stimulating treatments

Some injectable treatments work by encouraging your skin to produce more of its own collagen over several months. They do not give an instant result, and they suit people whose main concern is thinning skin and gradual laxity. I explain how they differ from filler in Collagen Biostimulators Explained.

Skin quality treatments

When the lines are fine and the skin is crepey, improving skin quality can make a visible difference. Options include microneedling with infusion and skin boosters. These usually need a course of treatments, and they improve texture rather than deep folds.

Skincare and sun protection

Daily broad-spectrum sunscreen and a well-chosen retinoid support collagen and slow further change. Skincare will not remove an established fold, but it protects any in-clinic results.

Other options discussed in consultation

Some treatments for this area are prescription-only. Australian regulations do not allow clinics to promote these online, so if they may suit you, they are discussed in person during a consultation with a medical prescriber.

Risks and side effects

All injectable and skin treatments carry risks. Common short-term effects include swelling, redness, tenderness and bruising, which usually settle within days.

Less common risks of dermal filler include lumps, asymmetry, infection and allergic reaction. A rare but serious risk is filler entering a blood vessel, which needs urgent treatment. This is why it matters that your injector understands facial anatomy and has a plan for complications. We discuss all risks with you before any treatment, and you are welcome to take time to think it over.

Recovery and realistic expectations

Most clients return to normal activities on the same day after dermal filler, though swelling can take a week or two to settle. Collagen-stimulating and skin treatments develop gradually over weeks to months.

A realistic goal is softening, not erasing. Marionette lines are part of how your face moves. The aim is for you to look rested and like yourself, not to look "done."

Choosing a clinic for marionette lines treatment in Melbourne

A few things are worth checking before you book:

  • Registration. Search your practitioner on the AHPRA register.

  • A real assessment. A proper consultation should look at your whole face and medical history.

  • Honest advice. A good practitioner will tell you when a treatment is not right for you.

  • Complication planning. Ask what happens if something does not go as planned.

Frequently asked questions

Add these at the end of the post. In Wix, you can also add them as an FAQ block so Google can read them as questions and answers.

What causes marionette lines? Marionette lines usually develop from a mix of volume loss, bone changes in the jaw, declining collagen, downward muscle pull at the mouth corners, sun exposure and genetics.

Can marionette lines be treated without injections? Fine lines can improve with skin treatments such as microneedling, plus a retinoid and daily sunscreen. Deeper folds caused by volume loss usually respond less to skin treatments alone.

How long does marionette lines treatment last? It depends on the treatment. Dermal filler results commonly last several months to over a year, while collagen-stimulating and skin treatments build gradually. Your metabolism, lifestyle and the product used all affect longevity.

Is marionette lines treatment painful? Most people describe mild discomfort. Numbing cream can be used, and many dermal fillers contain a local anaesthetic.

How much does marionette lines treatment cost in Melbourne? Cost depends on the treatment and amount needed, which is worked out during your consultation. We explain all costs before any treatment goes ahead.

What is the difference between marionette lines and nasolabial folds? Nasolabial folds run from the nose to the mouth corners. Marionette lines run from the mouth corners down towards the chin. They often need different approaches.
